QUESTION: How do I treat Severe Psoriasis, huge red plaques and big silver plaques of Psoriasis on my Legs

I have been suffering with psoriasis and psoriatic arthritis now for 30 years. I am now in my early 50's. My legs have huge red plaques and big silver plaques.

I have been a number of treatments for the arthritis ranging from Nsaids to prednisone to gold salts to methotrexate and finally the biological Enbrel brought to me relief to my arthritis. This did nothing for the psoriasis.

Switching to Simponi also did nothing. I have a UVB lamp that though helps, does not get rid of it.

I have been a corticosteroid ointments and the vitamin D derivative ointments and also various combinations if the two.

About the only relief I get is to butter myself up with the steroid ointment and wrap up my legs in plastic for a few days.

Recently I have discovered Oxypor which is an 1% OTC coal tar liquid which when I use this in combination with the UVB lamp, brings about similar results as the steroid and plastic wrap. I come to find out that this has a name called the Goeckerman treatment.

I want to pursue this further, but I am already at the point where the UVB really reddens my good skin (sun burn) after 5 minutes. I figure that I could use a stronger coal tar application with the same UVB lamp time and improve my results without the burn.

In my opinion the tar preparations are much kinder to the skin than the steroids under plastic wrap which tends to cause skin thinning which is not reversible if used long term.

There can be benefits in alternating the two treatments from time to time if the skin stops responding to one treatment you can go back to a previous treatment that you found effective in the past.

The Double Strength Psorexederm is a dark yellow/brown soothing ointment that spreads on easily and penetrates and protects the skin reducing scale very rapidly and lessening plaque. We get good results and I will be cautiously optimistic and say I would expect around a 25% improvement with a high likelihood of total elimination of the scale.
